Appointment changes

We are currently making changes to our appointment system.

This means in April 2019 we will only be offering on the day appointments.

We appreciate your patience and understanding while we make these changes, and we will update you when the changes have been completed. 

Extended access appoinments

GP and Nurse appointments are now available in your area during the evening and at the weekend.  To book an appointment, contact your Practice.  For more information, visit bit.ly/ExtendedAccess

Adult with cystitis symptoms?

If yes, please complete this questionnaire  and hand it in with a urine sample (in a labelled, small and closed container) at reception. Please do NOT hand the sample to reception staff but place in the designated receptacle.

Missed Appointments

A total of 115 appointments were wasted in March 2019, totalling 24 hours:
- 53 GP appointments (equivalent to 9 hours)
- 62 nurse and midwife appointments (equivalent to 24 hours)
If you are unable to attend an appointment please contact the surgery to cancel it - this time wasted could have been offered to other patients.

 

 

Pre-registration with the practice

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. 

This questionnaire can be downloaded by clicking on the link below. Alternatively, you can collect one from reception.

Please bring the completed and signed form to your new patient health check with the nurse.

Please note that completing the registration form does not guarantee that you will be registered as a patient with the practice. 

We will require proof that you are eligible for NHS treatment and that you live within our practice catchment area.
