FLU CLINICS:  Please note that we are now able to offer appointments for Flu vaccinations.  Saturday appointments are available on 12th October and 26th October.  Please contact Reception to book your appointment, or to check your eligibility to receive this vaccination.

Extended access appoinments

GP and Nurse appointments are now available in your area during the evening and at the weekend.  To book an appointment, contact your Practice.  For more information, visit bit.ly/ExtendedAccess

Adult with cystitis symptoms?

If yes, please complete this questionnaire  and hand it in with a urine sample (in a labelled, small and closed container) at reception. Please do NOT hand the sample to reception staff but place in the designated receptacle.

Missed Appointments

Patients did not attend for a total of 139 appointments in September2019, which is equivalent to 29 hours of wasted time:
- 61 GP appointments (equivalent to 13 hours)
- 78 nurse, midwife and flu injection appointments (equivalent to 16 hours)
If you are unable to attend an appointment please contact the surgery to cancel it, so that it can be offered to another patient.

The overall aim of the PPG is to develop a positive and constructive relationship between patients, the Practice and the wider community, ensuring that the practice remains accountable and responsive to its patients needs.

Patient Participation Group Membership

All patients who have been registered with the Practice for six months are welcome to join the PPG.

In the interests of effective decision making, a core Patient Participation group has been established, which usually meets every other month. A wider Patient Consultative Group (PCG) also exists for the core group to canvass for views and suggestions.

 

If you are interested in joining either group, you can pick up a form from reception or from your doctor. Alternatively, you can download a form by clicking on Membership Enquiry Form.
