If yes, please complete this questionnaire  and hand it in with a urine sample (in a labelled, small and closed container) at reception. Please do NOT hand the sample to reception staff but place in the designated receptacle.

Missed Appointments

Patients did not attend for a total of 130 appointments in December 2019, which is equivalent to 32 hours of wasted time:
- 55 GP appointments (equivalent to 13 hours)
- 102 nurse appointments (equivalent to 19 hours)
If you are unable to attend an appointment please contact the surgery to cancel it, so that it can be offered to another patient.

  • Our staff have strict rules of confidentiality and no personal details will be divulged without written consent.
  • Computer-held information is subject to the Data Protection Act. Medical information that cannot be traced to individual patients may, however, be passed on to third parties for research or statistical purposes. A patient may ask to see their medical record under certain conditions – please contact the Practice Manager for details.
  • We have a Practice Complaints Procedure to deal with comments, suggestions and complaints about the services we offer. Our practice manager can give you further information. Our aim is to give you the highest possible standard of service and we try to deal swiftly with any problems that might occur.
  • Patients have a responsibility to attend appointments or cancel in good time. We have a policy of removing from the list patients who persistently fail to attend.
  • Zero Tolerance

    Following a disturbing increase in verbal and abusive behaviour towards our staff in recent months, please be reminded that our staff and doctors are here to help you, and should not expect to be abused by anyone during the course of their work.

    We now write to all offenders to warn them that if their abusive behaviour does not change we will make arrangements to remove them from our patient list.

    We respect our staff, and take their Health and Safety in the workplace seriously.  Abusive behaviour will not be tolerated.
