Kirsty, our health and wellbeing coach can help you life a healthier life and support any patients living with the following:

  • Exercise Pathways
  • Respiratory Issues
  • Smoking Cessation
  • Food Diaries
  • 6 Week support for lifestyle, diet, exercise and behavioural changes

Social Prescribing Link Workers

Chelsie, Sue and Fiona, our social prescribers can help you with a large range of social and clinical needs either face to face or over the phone. This includes:

  • Addiction
  • Housing and Legal Affairs
  • Autism and Other Neurodivergences
  • Dementia Care
  • Carers Support
  • Food Bank
  • Bereavement
  • Loneliness

The list goes on!

For both professionals and members of the public, it is worth getting in touch with your local social prescriber, accessed through the GP or GP reception team, to see if they can support you, a family member, or your patients (professionals).

Care Co-Ordinator

Charlotte, our care co-ordinator, helps patients by:

  • Following patients through their cancer journey - from referral to post treatment support
  • Building care plans for care home residents
  • Completing lifestyle referrals, e.g Weight management and smoking cessation
  • Signpost carers to support
  • Help Mental Health patients access support
  • Supporting research Projects
